Urgent / Priority Referrals
Prior to processing referral, it is important to screen referral information and escalate any concerns to your Concierge Manager to determine if escalation to the relevant Business Partner or Clinical Lead is required. Although the word ‘urgent’ may be used by the referrer, there are many cases that do not warrant prioritisation above other referrals, so please only escalate files that have genuine risk to safety or wellbeing involved.
Eligible circumstances may include:

Hospital to home referrals: This is a priority focus area for the NDIA, so please ensure that these referrals are processed in a timely manner and raised with the Concierge Manager

Behaviours of concern: If the referral references a significant recent change in behaviours, ensure that this referral is processed in a timely manner and raised with the Concierge Manager

Significant swallowing difficulties: As this can present an immediate risk to participant safety without our assessment and intervention, ensure that this referral is processed in a timely manner and raised with your Concierge Manager

Diagnosis of MND or similar rapidly degenerative disease: This is a priority focus area for the NDIA, and can present an immediate risk to participant wellness and this may require an immediate AT response

Safety concerns: If there are significant safety concerns that could be a risk to the participant or our clinicians, raise these with the Concierge Manager prior to proceeding
